Please fill in the questionnaire that is relevant to your set of circumstances.

Please fill in just one questionnaire, even if you are both a competitor and spectators, I would ask you just to fill in one, as this will taint the findings.

Each questionnaire will have instructions on how to complete the questionnaires.
